After More Than a Decade in Darkness, Groundbreaking ‘Tooth-in-Eye’ Procedure Restores Sight to 75-Year-Old Woman
For the first time in over a decade, 75-year-old Gail Lane from Victoria, British Columbia, can see the world once again—including the face of her partner and the wagging tail of her dog—thanks to a remarkable and rare surgical procedure that uses her own tooth as a structural support for an artificial cornea.
The Remarkable Return of Sight: Case Background
Gail Lane is among only a handful of individuals in Canada—and one of just three in the country—who have undergone osteo-odonto keratoprosthesis (OOKP), a highly specialized surgery designed to restore vision in patients who are not candidates for conventional corneal transplants. This advanced procedure was performed in February at Vancouver’s Mount Saint Joseph Hospital by renowned ophthalmologist Dr. Greg Moloney, who pioneered the technique’s introduction to Canada. Previously, similar surgeries had been carried out in select medical centers worldwide, often as a last hope for people with deep-seated and irreversible corneal blindness.
Lane’s journey to blindness began 11 years prior, after an autoimmune disease left her corneas permanently scarred and opaque. Conventional corneal transplants were not feasible due to the severity of her ocular surface disease, making the OOKP procedure her only remaining hope.
Understanding Osteo-Odonto Keratoprosthesis: What is ‘Tooth-in-Eye’ Surgery?
Osteo-odonto keratoprosthesis is considered one of the most complex and unusual ocular surgeries in the world. Developed in Italy in the 1960s by Professor Benedetto Strampelli, it is reserved for the most challenging cases of corneal blindness, particularly where the surface of the eye has been damaged by trauma, burns, or as in Lane’s case, autoimmune disorders.
The OOKP procedure is completed in multiple stages and requires immense skill and coordination between ophthalmic surgeons and maxillofacial specialists. Dr. Moloney described it as “complex and strange,” explaining that the process essentially replaces the patient’s damaged cornea with a unique structure constructed from the patient’s own tooth and the adjacent bone.
Step-by-Step: How the Tooth-In-Eye Procedure Works
The intricate surgery begins by harvesting one of the patient’s own teeth—usually a canine tooth together with some bone. The tooth serves as a biological ‘frame’ or carrier for a small, clear plastic lens, which will ultimately serve as a window to the outside world.
- The chosen tooth is surgically extracted, along with a small block of the surrounding jawbone.
- The tooth-bone segment is carefully shaped and drilled to accommodate a tiny transparent cylindrical lens.
- This tooth-lens construct is not immediately implanted into the eye. First, it is temporarily embedded under the skin—typically in the patient’s cheek—to allow the body’s tissues to accept it and provide vascularization. This step can take several months.
- In the final stage, eye surgeons create a space in the patient’s eye, remove the scarred cornea, and implant the living tooth-lens structure into the eye socket. The tooth and bone provide a biological, robust, and biocompatible support for the optical cylinder, integrating with surrounding tissues and reducing risk of rejection.
According to Dr. Moloney, “We need a structure strong enough to hold on to the plastic focusing telescope, but not one that the body will reject.” The unique use of the patient’s own mouth tissue dramatically lowers immunological risk compared to synthetic alternatives.
Patient Experience: From Darkness to Detail
Lane described the entire process as “uncomfortable but not painful.” After years of total darkness, she experienced the gradual return of vision in distinct stages: first perceiving light, then recognizing movement, and finally, patterns and details emerged. Slow and patient recovery is typical, as the brain must relearn to process visual information after such a lengthy period without sight.
“It’s been a long wait, but it was well worth it,” Lane said. As her eyes and brain adjusted, familiar shapes began to reappear in her world. The first moments she recognized her partner’s service dog, Piper—a black Labrador—filled her with joy. Soon after, colors, shapes, and the fine textures of nature—like grass, flowers, and trees—returned to view.
Psychological and Social Impact: Seeing Loved Ones Again
One of the most profound emotional experiences for Lane came several months after the procedure when she laid eyes on her partner, Phil, for the first time. Having met and established their relationship after she had already lost her vision, the revelation of his facial features was a deeply meaningful breakthrough.
“I’m starting to see facial features on people, which is also pretty exciting,” she shared, highlighting the powerful human connection that vision can offer. She hopes that with new glasses, she might even be able to see her own face clearly in the mirror in the coming months—a milestone that underscores the life-changing magnitude of the surgery.
Regaining Independence and Daily Life Skills
Before the OOKP procedure, Lane relied heavily on assistive technologies to navigate daily tasks. For instance, she frequently used an app called ‘Be My Eyes,’ which connects visually impaired users with volunteers to help with tasks like matching clothing. Now, Lane enjoys greater independence and confidence in choosing her outfits and re-engaging with her surroundings.
“I’m hoping to have more mobility and independence for short trips and walks here and there, where I don’t always have to have someone’s arm to hold onto,” she expressed. This newfound autonomy is a testament not only to the surgery’s technical success but also to its transformative effect on quality of life, self-esteem, and personal agency.
Expert Analysis: Who Benefits from Osteo-Odonto Keratoprosthesis?
Conventional corneal transplants are highly successful in the majority of cases where the ocular surface is healthy, but for patients with severe corneal damage and hostile ocular environments, success rates plummet. The OOKP process offers hope to those with intractable blindness secondary to:
- Severe chemical or thermal ocular burns
- Autoimmune diseases like Stevens-Johnson syndrome or mucous membrane pemphigoid
- Herpetic infections that have destroyed the eye’s surface
- Failed previous corneal transplants
Dr. Moloney’s team were able to select Gail Lane as an ideal candidate due to her overall health, detailed medical evaluation, and her unwavering resolve. Worldwide, OOKP is still rare—undertaken only in specialized centers with access to both ophthalmic and dental expertise. In Canada, Dr. Moloney’s introduction of the procedure stands alongside successful programs in countries such as the United Kingdom, Italy, India, and Australia.
Procedural Risks, Limitations, and Ethical Considerations
While the tooth-in-eye surgery represents a breakthrough, it is not without risks or limitations. Surgery is multi-staged and lengthy, with not every candidate eligible due to overall health or structural issues. Postoperative management is complex, and patients must be closely monitored for complications including infection, extrusion of the device, or resorption of the tooth-bone implant. The vision achieved, although remarkable, is often limited to central visual acuity and not as sharp as that from a healthy, natural eye.
Ethical considerations include the rigorous informed consent process, particularly as this is a last-resort treatment. Involving experts from both dentistry and ophthalmology, as well as social workers and psychologists, ensures patients’ expectations are grounded in medical reality.
Global Context and Research Developments
Globally, the OOKP technique is being studied and refined as researchers seek to make the procedure safer, more widely available, and easier to perform. Some developments focus on artificial materials or donor bone to avoid dental surgery, while others refine the optical cylinder for improved vision quality. Teams worldwide continue to monitor long-term outcomes, confirming that the tooth-in-eye approach can deliver durable, years-long restoration of vision for previously untreatable patients.
Societal responses to such stories often mix awe at medical innovation with admiration for patient resilience. The re-integration of patients like Lane into their communities serves not only as a beacon of hope for others living with blindness but also as a call for expanded investment in advanced ophthalmic research and healthcare access.
The Future of Vision Restoration
Lane’s story is emblematic of the cutting edge of modern medical science, where interdisciplinary collaboration and patient-centered care meet to deliver unimagined transformations. While the tooth-in-eye procedure is not suitable for every form of blindness, it stands as a beacon for those facing total vision loss after exhausting conventional treatments.
The future may see continued advances, including engineered materials that replicate the success of OOKP without the need for tooth extraction, as well as less invasive alternatives. For now, however, the courage of patients like Gail Lane and the dedication of surgical pioneers mark a new chapter in restoring the miracle of sight.
